State inspectors began looking into the FedEx Freight hub in July, said Russell Perry, head of the Iowa Occupational Safety and Health Administration. The investigation is based on an employee complaint that the company had not provided proper personal protective equipment to workers after a spill of formic acid. Since then, employees say two similar incidents have occurred at the hub at 5890 N.E. 22nd St. in Polk County’s Saylor Township.
